What is the Carers Hospital Discharge Project?

Carers Manchester has received funding* for a 12-month project focusing on supporting unpaid carers during and after the hospital discharge process of the cared-for. Providing support at this stage is important to reduce carer burnout and ensure carers are equipped to safely continue looking after the person they care for when they return home.

The project has 3 main aims:

  • Empowering unpaid carers during the cared-for’s hospital discharge process
  • Reduced length of stay for patients clinically ready for discharge ​
  • Reduction in re-admission into hospital

Each of the hospitals have a new Carers Hospital Discharge Worker: Courtney in Manchester Royal Infirmary, Cathy in Wythenshawe Hospital, and Emma in North Manchester General Hospital.

The team will work with staff and carers to help raise awareness about the role of carers and the extensive support available, including emotional support, support in the community, referrals for Carer’s Assessment to Manchester City Council and information about carers’ rights.
